  • For Meal Boxes, you may be eligible if you:

    • Use emergency or hospital services often

    • Are in a NYS Health Home with conditions like HIV, serious mental illness, or multiple chronic conditions

    • Have a substance use disorder or developmental disability

    • Are pregnant or recently postpartum

    • Were recently released from incarceration with a chronic condition

    • Are a high-risk child or youth, including those in foster or kinship care

    For Grocery Boxes, you may be eligible if you:

    • Are pregnant or recently postpartum

    • Are a high-risk child or youth, including those in foster or kinship care

    • Reside in a household with someone that falls into one or both of the above categories 

  • Medicaid members are encouraged to apply to assess eligibility.

  • Our dietitian crafts meals and chooses grocery ingredients to support a variety of chronic conditions. Please reach out to a team member and we will discuss some of our sample options.

  • All options are heart-healthy and diabetic-friendly. Accommodations can be made for vegetarian/vegan diets. Halal and Kosher options are available.
