  • You may be eligible if you:

    • Use emergency or hospital services often

    • Are in a NYS Health Home with conditions like HIV, serious mental illness, or multiple chronic conditions

    • Have a substance use disorder or developmental disability

    • Are pregnant or recently postpartum

    • Were recently released from incarceration with a chronic condition

    • Are a high-risk child or youth, including those in foster or kinship care

  • Medicaid members are encouraged to apply to assess eligibility.

  • Our menus are crafted to be heart-healthy, diabetic-friendly, and halal-certified. Accommodations are available for allergens and vegetarian/vegan diets. Kosher options are expected soon.
