You may be eligible if you:
- Use emergency or hospital services often
- Are in a NYS Health Home with conditions like HIV, serious mental illness, or multiple chronic conditions
- Have a substance use disorder or developmental disability
- Are pregnant or recently postpartum
- Were recently released from incarceration with a chronic condition
- Are a high-risk child or youth, including those in foster or kinship care
