You may be eligible if you:
- Use emergency or hospital services often
- Are in a NYS Health Home with conditions like HIV, serious mental illness, or multiple chronic conditions
- Have a substance use disorder or developmental disability
- Are pregnant or recently postpartum
- Were recently released from incarceration with a chronic condition
- Are a high-risk child or youth, including those in foster or kinship care
Medicaid members are encouraged to apply to assess eligibility.

Yes, meals are fully covered for eligible members under Medicaid's 1115 Waiver.
We are currently serving Manhattan, Brooklyn, Queens, the Bronx, and Long Island. We expect Staten Island to be included in the near future.
